Most of our care is delivered through outpatient clinics/ day-case admissions. However when we do have inpatients they can be very sick with infection or multi-organ disease. Many Paediatric Rheumatology patients receive immunosuppressive treatments, but not all are very vulnerable to infection. General medical paediatric team may be asked to cannulate or review Paediatric Rheumatology patients before IV infusions given as day-cases. The majority of these infusions take place with the day unit nurses running everything. If you are concerned that a child may have a significant infection (anything other than an a febrile URTI with conserved appetite) then contact one of the consultants before starting the infusion.Plan for infusions for each patient are kept in cardboard folder on day-unit. While placing IV cannula please send blood for FBC, ESR, CRP, U+Es and LFTs (as well as anything else requested on drug chart or in cardboard folder). Paediatric Rheumatology inpatients are shared with General Paediatric team, with general paediatric junior staff very involved in the care. The Paediatric Rheumatology consultants are not paid to work out of hours, but are happy to be called (via LGI switchboard) if the on-call Consultant General Paediatrician advises or if they have specifically requested to be called, for a specific inpatient that they are caring for. Clinic letters are usually copied to families, so they may bring letters if need to attend as an emergency. We will hopefully have a system to allow access for all Paediatricians to patient letters on I drive, but not available yet: watch this space!
